Two reflections on the title :
1) Should it be Doctors ?
2) For many, many years fans only have to say the word "Day" when talking about stories and you know it's the first Pertwee Dalek story being referred to. We won't be able to do that now!

The only other lead I have is it's Michael Keating's character from "The Sunmakers" but even though I wouldn't think twice about making an obscure reference to a one off guest appearance in a trailer... it probably isn't him either!

I've seen it suggested that maybe it's the Brigade-Leader from Inferno? It's a good match I guess (shirt, trousers, and 'sash') but an odd choice.

One thought that occurred to me today, and since I'm avoiding any and all spoilers it is just an idle thought... if Hurt's Doctor is a 'missing' incarnation somewhere along the line, then presumably Matt is Doctor 12, David is Doctor 11, etc, etc. Which puts quite a different spin on all this "at the Fall of the Eleventh" business doesn't it...?

Possibly although I think the point "The Name of the Doctor" was hinting at is that there have only been 11 incarnations who used the name "Doctor". The Hurt incarnation operated using the Doctor's actual name, or at least that was implied by the reference in the Time War book in "Journey to the Centre of the TARDIS" and more importantly by the final line "But you didn't do it in the name of the Doctor!" So I believe "The Fall of the Eleventh" which may even be the same even as "The Day of the Doctor" still refers to the Smith incarnation.
I suspect the fact that the Doctor has possibly only one more regeneration left will be addressed in the Christmas special especially if the rumoured title is correct.
