    Default Doctor? Doctor Who?

    "Doctor? Doctor who?"

Just how many times has the above gag (or a variation of it) been used in our programme and can we name all of the times it happens?
And yes I know its the title of the programme, smarty pants, so you can't have that as an example.

Don't forget The Christmas Invasion, where Jackie utters it.

Unearthly Child. Episode 2 I believe? And yes that's what I call the story and no I'm not changing my mind

The Doctor himself says it and then Ian says it too a little later.

'Just open the doors, Doctor Foreman.'
'Eh? Doctor who? What's he talking about?'
The Doctor and Ian, The Cave Of Skulls.

'That's not his name. Who is he? Doctor who?'
    Ian, about 5 minutes later.

(probably not really qualified as gags, since at this point the question was legitimate, and in fact being set up in those exchanges)

    'I am Doctor Caligari.'
    'Doctor who?'
    'Yes, absolutely right.'
The Doctor and Wyatt Earp, The Gunfighters.

Not gags as such but who can forget "Doctor Who is required" from The War Machines and Dr.W in "The Mary MacMillian Menace" and Doctor Von Wer in "The Highlanders".

It's like TV Comic come to life if only the Dr's car Betsy was around...
