I was thinking more along the lines of exactly when he woke up...could it possibly have been before he took Clara in search of Danny meaning that much of the final two episodes of S8 could have all been a dream? I can't think of any reason why the Doctor would return there...

But it wasn't actually a real location in Dark Water was it? That was a 'dream' environment created in Clara's mind inside the TARDIS. Maybe it just left the Doctor with a hankering to visit a volcano - maybe he was getting nostalgic for the good old days with the Brigade Leader and Section Leader Elizabeth Shaw...

The volcano scene was shot on a slightly redressed set from Wizards vs Aliens so it would've been very easy for them to have re-used it for Last Christmas as well.
They certainly got their money's worth out of North Pole base set. Could've done with a bit less standing around talking in some of those scenes and more Alien pastiche.

It has just dawned on me though (a little belatedly) that if the Doctor encountered Santa in his TARDIS at the end of Death In Heaven, then he must already have been dreaming...for how long, exactly, at that point?

Obvs Death In Heaven was all a dream! Pretty much all the way back to Dark Water. Everything from the scene with Clara and the TARDIS keys onwards never happened.
